Appointment Scheduling

Please fill out the forms below before you visit. It is important that you list out your current medications including any over the counter medications that you may be taking. Please arrive to the appointment at least 15 minutes in advance if you have the paperwork filled out or 30 minutes in advance in you have not been able to fill out the paperwork. Please also bring a copy of your insurance card with an ability to pay your co-pay. The first session will last from 1 to ½ hours and will consist of the reason for your treatment as well as collecting psychosocial history.

Required Patient Forms

New patients should arrive 30 minutes prior to the appointment time so that we can process the information, create the medical file and place the chart in line for the designated appointment time. If you have a computer and printer, you may download the required forms now. Bring all medications in their original bottles (not a pill box) to your appointment. Be sure to include any over-the-counter medications you current take.

Fees, Billing & Insurance

The Center For Mental Wellness accepts most insurance plans. Our staff will be happy to file the insurance for you. Your insurance coverage is an agreement between you and the insurance provider. However, the final responsibility for payment of the bill belongs to you. All co-pays are due at the time services are rendered. If you are not utilizing insurance coverage, the entire fee will be due at time of service. If you are unable to pay the entire fee please consult with the office manager for a payment arrangements before your initial session. If you are uncertain if insurance will pay, our office staff will help you through this process and let you know how much the evaluation cost if insurance will not cover.

Pay Your Bill Online

You can pay your bill online using Online Bill Pay.

Telephone Request

You can call the office to tell us why you or your child would like to be seen. The office manager will ask for your insurance information so we can determine if mental health benefits are covered by your insurance plan.

If they are not, we will let you know the fee for service.

Release of Information

We are unable to release any information to anyone without your written consent on the required Alternate Contact Form. Your information is privacy-protected.
